Process Overview

The laboratory processing of a drug test result includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the primary handling of a sample into a lab's system. This phase involves ensuring the sample was sealed and shipped accurately, assigning a unique LAN (Laboratory Accessioning Number), and completing any missing data entry not covered by an electronic chain of custody procedure.

However, if a positive screen occurs, it requires further verification for legal admissibility. Samples that return a presumptive positive in Screening must undergo an additional confirmation stage.

Here, drugs are separated from hair at much lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening particularly challenging to execute.

Confirmation of any positive results from the screening process is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are appropriately washed before confirmation. From Accessioning through Confirmation, the entire laboratory procedure is reviewed according to both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.
