If scalp hair is unavailable, an equivalent amount from body hair is permissible. "Head hair" specifies scalp hair exclusively, while "body hair" includes all other types (such as facial and axillary hair).

Process Overview

The four principal phases in the lab processing of a drug test include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially incorporating a sample into a lab's system. This step verifies that the sample was appropriately sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry not available via an electronic chain of custody system.

Screening provides a preliminary check for drug substances. While efficient for excluding drug presence in most instances, a positive screening requires confirmation for legal acceptance. Any samples presumed positive during Screening need subsequent verification.

For specimens presumed positive in the Screening phase, additional hair is taken from the initial sample and prepared for Extraction. This phase isolates drugs from hair at much lower concentrations than other methodologies (such as urine or oral fluid), making hair drug testing more complex.

Positive screening results are confirmed using techniques like GC/MS, GC/MS/MS, or LC/MS/MS. Before Confirmation, all presumed positive samples undergo washing as necessary. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
