What it is, How it works

Hair testing is recognized as an effective means for identifying drug and alcohol consumption. Hair captures a long-term record of alcohol and other substances by embedding biomarkers within the fibers of the expanding hair strand. When taken close to the scalp, hair can offer up to a 3-month detection window for alcohol and other drugs. Hair collection is straightforward, relatively resistant to tampering, and convenient for shipment.

A sample of 1.5 inches in length, comprising roughly 200 strands of hair (comparable in size to a #2 pencil) nearest to the scalp, provides 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, a 150mg specimen is advised. We suggest weighing the specimen with a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair can be collected. When mentioning head hair, we mean scalp hair exclusively. Body hair refers to all other hair types (facial, axillary, etc.).

Process Overview

The laboratory handling of a drug test result comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial setup of a sample within the laboratory system, including making sure the sample was properly sealed and shipped, assigning a randomly generated LAN (Laboratory Accessioning Number), and completing any additional data entry that isn't supplied by an electronic chain of custody system.

Screening acts as a preliminary inspection for drugs of misuse. While Screening is a cost-effective strategy to eliminate drug use on the bulk of samples, a screen testing positive requires confirmation to be permissible in court. Any samples yielding a presumptive positive in Screening must undergo secondary confirmation.

For a sample with a presumptive positive in the Screening phase, additional hair is extracted from the original specimen and prepared for Extraction. This process retrieves drugs from hair at a considerably lower concentration than with other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging technique to execute.

Confirmation of any positive screening outcome is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All presumed positive samples undergo washing before confirmation as necessary. The comprehensive laboratory process from Accessioning through Confirmation is assessed under the CAP (College of American Pathologists) Hair designation and authenticated against ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
