What it is, How it works

Hair testing is now considered an effective method for identifying drug and alcohol usage. Hair strands capture biomarkers of alcohol and drugs within their fibers as they grow, providing a long-term record. When taken near the scalp, hair can offer a detection window of up to roughly 3 months for alcohol and other substances. Hair is easy to collect, somewhat resistant to tampering, and convenient to send for analysis.

A 1.5-inch section of about 200 hair strands (approximately the thickness of a #2 pencil) close to the scalp amounts to 100mg of hair, the optimal quantity for screening and confirmation. For EtG, additional tests, and/or testing beyond 10 panels, a sample of 150mg is advised. It's best to weigh the sample on a jeweler’s scale. If scalp hair cannot be collected, an equal portion of body hair may be used. When we refer to head hair, we mean scalp hair exclusively. Body hair includes any other types of hair (like facial or axillary hair).

Process Overview

The laboratory handling of a drug test result consists of four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step in incorporating a sample into a lab’s system. This step involves confirming that the sample was correctly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing any needed data entry absent in an electronic chain of custody system.

Screening acts as an initial quick inspection for signs of drug abuse. While Screening is a cost-effective strategy to eliminate drug usage in most samples, any positive screen needs confirmation in order to be permissible in a legal setting. Samples that appear positive during Screening require further confirmation.

Should a sample show a presumptive positive in the Screening phase, additional hair is extracted from the original sample for the Extraction process. In this phase, drugs are removed from hair at a significantly lower concentration compared to other methods (such as urine or oral fluid), making hair drug analysis the most complex method to carry out.

Confirmation of any positive screening outcome is achieved through GC/MS, GC/MS/MS, or LC/MS/MS techniques. Samples deemed presumptively positive undergo a washing process prior to confirmation requirements. The entire lab procedure, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 specifications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
