If scalp hair is not available, a comparable quantity of body hair may be obtained. The term head hair refers specifically to scalp hair, while body hair encompasses all other types (such as facial and axillary hair).

Process Overview

The laboratory processing of a drug test outcome encompasses four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step, integrating a sample into a lab’s system. This stage confirms that the sample has been adequately sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and addresses any extra data logging not managed by an electronic chain of custody arrangement.

Screening executes an initial rapid evaluation for drugs of abuse. While Screening is a budget-friendly approach to eliminate drug presence in most samples, a positive screening result must be substantiated to be acceptable in legal contexts. Samples flagged as presumptively positive during Screening require additional verification.

If a sample registers as presumptively positive during Screening, additional hair is extracted from the initial sample for preparation in the Extraction phase. At this juncture, the drugs are isolated from the hair at a lower concentration compared to other approaches (such as urine or oral fluid testing), accounting for the complexity of hair drug screening.

Confirmation of any positive screening outcome is conducted utilizing GC/MS, GC/MS/MS, or LC/MS/MS. All samples preliminarily deemed positive are cleansed before confirmation as necessary. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
