A 1.5-inch segment containing about 200 strands (roughly equivalent to a #2 pencil) nearest the scalp provides 100mg of hair, which is the optimal sample for both screening and confirmation. For EtG, additions, and/or tests exceeding 10 panels, a 150mg sample is advisable. It's suggested to measure the sample using a jeweler’s scale. If scalp hair cannot be obtained, an equivalent quantity of body hair may be taken. When mentioning head hair, it pertains solely to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial entry of a sample into a laboratory’s system, which includes ensuring the sample was sealed and dispatched correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent in an electronic chain of custody system.

Screening comprises a preliminary rapid assessment for drug abuse. While Screening serves as an economical means to exclude drug use in most samples, a positive screen must be confirmed to hold up in court. Any samples suspected of being positive during Screening necessitate a secondary confirmation.

If a sample is considered positive in the Screening stage, more hair is extracted from the initial sample and prepared for Extraction. At this stage, drugs are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), marking hair drug screening as the most challenging methodology to perform.

Confirmation for any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumptively positive are washed before confirmation, as required. The complete laboratory process from Accessioning to Confirmation undergoes a review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
