What it is, How it works

An ideal screening and confirmation sample consists of a 1.5-inch segment of around 200 hairs (analogous to the diameter of a #2 pencil) nearest to the scalp, which equates to 100mg of hair. For EtG, additional tests, or panels beyond 10, a 150mg specimen is advised. It's recommended to weigh samples on a jeweler’s scale. If scalp hair isn't present, an equivalent quantity of body hair can be used. When mentioning head hair, it's specifically scalp hair. Body hair denotes other types (like facial, axillary, etc.).

Process Overview

The laboratory procedure for drug test results encompasses four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial integration of a sample into the lab’s processes. This includes checking the sample’s seal and shipment, generating a LAN (Laboratory Accessioning Number), and fulfilling extra data entry not covered by an electronic system for custody tracking.

Screening acts as a preliminary evaluation for drugs of abuse. While it economically excludes drug use on many samples, a positive finding must be confirmed for legal admissibility. Presumptive positives must undergo secondary confirmation.

Confirmation of any positive screening findings is done through GC/MS, GC/MS/MS, or LC/MS/MS analysis. Presumptive positives undergo washing before confirmation as needed. Every procedural aspect from Accessioning to Confirmation is appraised under the guidelines of both the CAP (College of American Pathologists) Hair certification and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
