What it is, How it works

When acquired near the scalp, hair may reveal about a 3-month detection range for alcohol and drugs. Hair is easy to collect, relatively hard to tamper with, and simple to transport.

A 1.5-inch sample consisting of roughly 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal sample size for analysis and validation. For EtG, additional components, and/or tests exceeding 10 panels, a 150mg specimen is advised. It's suggested to measure the specimen using a jeweler’s scale. If scalp hair is absent, a comparable amount of body hair can be used. Reference to head hair implies only scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory process of a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a sample into a lab's system. This step involves verifying the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any additional data entry not covered by an electronic chain of custody system.

Screening tests for drugs of abuse through an initial rapid check. Screening is a cost-efficient method to eliminate drug use in most samples, but a positive screen necessitates confirmation to be court-viable. All presumptively positive Screening samples require a secondary confirmation.

If a sample shows a presumptive positive in Screening, additional hair is extracted from the original sample for Extraction preparation. During this phase, drugs are extracted from hair at a significantly lower concentration than in other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive Screening outcome is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed prior to necessary confirmations. The entire laboratory process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
