Hair collection is straightforward, relatively tamper-resistant, and easy to transport.

A 1.5-inch section comprising around 200 hair strands (resembling the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the preferred sample size for testing and verification. For EtG tests, add-ons, and tests exceeding 10 panels, 150mg of hair is advised. We suggest weighing the sample using a jeweler’s scale. If scalp hair is unavailable, a comparable quantity of body hair may be utilized. When referencing head hair, we mean scalp hair specifically. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The four central stages in the laboratory analysis of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning engages the initial integration of a sample into a laboratory’s database. This step includes confirming that the sample was securely sealed and dispatched correctly, generating a random LAN (Laboratory Accessioning Number), and finalizing any data input not available through an electronic chain of custody system.

Screening comprises a preliminary swift examination for drugs of abuse. Although Screening is a cost-effective technique to exclude drug usage in most samples, a positive result necessitates confirmation to be considered valid in legal proceedings. Samples tentatively positive in Screening must undergo further confirmation.

Should a sample exhibit a presumptive positive during the Screening phase, additional hair is extracted from the initial sample and prepped for Extraction. In this phase, drugs are extracted from hair at much lower concentrations than alternative methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive result from Screening is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples undergo washing as needed. The complete lab process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
