What it is, How it works

Hair captures a long-lasting history of substances by encasing biomarkers within the fibers of the emerging hair strand. When taken near the scalp, hair can offer approximately a 3-month detection window for alcohol and drugs. Hair is easy to collect, somewhat hard to tamper with, and simple to transport.

A 1.5-inch section consisting of around 200 strands of hair (about the size of a #2 pencil) nearest to the scalp provides 100mg of hair, the optimal sample for analysis and verification. For EtG, additional tests, and/or over 10 panels, 150mg of the sample is suggested. We advise measuring the sample on a jeweler’s scale. If scalp hair isn't available, a comparable amount of body hair can be gathered. When talking about head hair, we mean scalp hair exclusively. Body hair includes all other forms of hair (facial, axillary, etc.).

Process Overview

The four primary stages in lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial handling of a sample into a lab’s system. This step includes ensuring the sample was sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entries not supplied by an electronic chain of custody system.

Screening is a preliminary quick assessment for drugs. Although Screening is an economical way to rule out drug usage for most samples, a positive screen must be confirmed to be valid in court. Any samples that are presumptively positive in Screening necessitate a further confirmation.

If a sample is presumptively positive in the Screening stage, additional hair is taken from the initial specimen and prepared for Extraction. In this phase, drugs are isolated from hair at a substantially lower concentration than in other techniques (e.g., urine or oral fluid), resulting in hair drug screening being the most complex methodology to conduct.

Confirmation of any positive screening result is carried out via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are washed before confirmation as required. The entire lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
