Over the years, hair follicle drug testing has become a more common method for drug screening. Employers, legal systems, and substance abuse experts are increasingly opting for hair tests in lieu of standard urine tests. Hair analysis is increasingly recognized as an effective method for identifying drug and alcohol consumption. Hair captures a prolonged record of alcohol and various substances by embedding biomarkers within the hair's growing fibers. When obtained near the scalp, hair can show a detection timeframe of up to around three months for alcohol and other substances. Hair is straightforward to gather, relatively hard to modify, and simple to transport.
A sample measuring 1.5 inches, containing roughly 200 hair strands (similar in size to a #2 pencil) nearest to the scalp, will yield 100mg of hair, which is a perfect quantity for evaluation and validation. For EtG, additional services, and/or tests exceeding 10 panels, a 150mg specimen is advisable. We recommend utilizing a jeweler’s scale for specimen measurement. If head hair is not accessible, a comparable quantity of body hair might be acquired. When we mention head hair, we specifically refer to scalp hair. Body hair includes all other hair types (such as facial and axillary).
Process Overview
The four primary stages in the laboratory evaluation of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.
Accessioning pertains to the preliminary treatment of a specimen within a laboratory’s procedure. This process verifies that the sample was appropriately sealed and transported, allocates a random LAN (Laboratory Accessioning Number), and fulfills any additional data entry absent in an electronic chain of custody process.
Screening signifies an essential initial assessment for substances of abuse. While benefiting as a cost-efficient way to exclude substance use in most specimens, a positive screen necessitates confirmation to be valid in legal proceedings. Specimens shown as presumptively positive in Screening necessitate further confirmation.
If a specimen is ruled presumptively positive at the Screening phase, additional hair is taken from the main sample and readied for Extraction. During this stage, drugs are withdrawn from hair at notably lower concentrations than with alternative methods (like urine or oral fluid), illustrating why hair drug assessment is the most challenging methodology.
Verification of any positive screening outcome is implemented via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if required. The complete laboratory process from Accessioning to Confirmation undergoes inspection under both the CAP (College of American Pathologists) Hair classification and accreditation in compliance with ISO / IEC 17025 directives.

Note: While commonly called "hair follicle tests", the analysis is performed on the hair strand and not on the follicle beneath the scalp.
