The laboratory processing of a drug test comprises four integral stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary processing of a sample into a laboratory's system. This process confirms the sample was properly sealed and despatched, allocates a random LAN (Laboratory Accessioning Number), and executes any supplemental data entry not handled by an electronic chain of custody system.

Screening involves a swift preliminary check for drugs of abuse. Although Screening offers a cost-effective method to dismiss drug use in most samples, a positive screen must be confirmed to hold legal credibility. Any samples testing positive during Screening necessitate further confirmation.

In instances of a positive Screening outcome, additional hair is extracted from the original sample for the Extraction phase. Here, drugs are extracted from hair at diminutive concentrations compared to other methods (ex. urine or oral fluid), making hair drug analysis particularly intricate.

Confirmation of any positive screening is conducted through GC/MS, GC/MS/MS or LC/MS/MS. Presumptive positive samples are washed prior to confirmation if needed. The full laboratory procedure, from Accessioning to Confirmation, is supervised under both the CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 standards.
