For tests involving EtG, additional screenings, or those exceeding 10 panels, a 150mg specimen is advised. It is recommended to weigh the sample on a jeweler’s scale. If scalp hair is not accessible, an equivalent amount of body hair can be collected. Reference to head hair specifically pertains to scalp hair. Body hair denotes hair from other parts (such as facial, axillary, etc.).

Process Overview

There are four primary stages in the laboratory analysis of a drug test: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial handling of a sample in a lab’s protocol. This process involves confirming proper sealing and transport of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any data entry absent from an electronic chain of custody system.

Screening is a preliminary procedure to detect possible drug presence. Any samples testing presumptively positive in Screening require an additional confirmation step.

If a sample tests presumptively positive during Screening, additional hair from the original sample is taken for Extraction. Here, drugs are isolated from hair at much lower concentrations than other methods (e.g., urine or saliva), which is why hair drug testing presents significant challenges.

Confirmation of any positive result from Screening is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Before confirming, all samples that tested presumptive positive are washed as needed. The entire process, from Accessioning to Confirmation, adheres to both the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 accreditation.
