A hair sample of 1.5 inches containing around 200 strands (similar in size to a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, additional tests, and/or tests over 10 panels, 150mg is suggested. It’s advised to weigh the specimen on a jeweler’s scale. In the absence of scalp hair, an equivalent amount of body hair may be obtained. By head hair, we mean scalp hair specifically. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The primary steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step where a sample is processed into a lab’s system. This involves confirming that the sample was sealed and transported properly, assigning it a unique LAN (Laboratory Accessioning Number), and completing necessary data entry not included by an electronic chain of custody.

Screening is a preliminary swift assessment for drugs of abuse. While Screening is an economical method to rule out drug use in most samples, a positive screen needs confirmation to be viable in legal settings. Samples that are presumed positive in Screening require further confirmation.

For samples presumed positive at the Screening phase, additional hair is taken from the initial sample for Extraction. During this phase, drugs are extracted from hair at lower concentrations than other methods (e.g., urine or oral fluid), making hair drug testing one of the challenging techniques to perform.

Any positive screening result is confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are washed suitably before confirmation. The entire lab process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair certification and aligned with ISO / IEC 17025 standards.
