For EtG, additional tests, and/or more than 10 panels, a sample of 150mg is advisable. Use a jeweler’s scale for accurate weighing. If scalp hair isn’t available, a similar amount of body hair can be used. Head hair refers exclusively to scalp hair while body hair encompasses other types like facial and axillary hair.

Process Overview

There are four primary phases in processing a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves incorporating a sample into a lab’s system, including checking that it was sealed and shipped correctly, allocating a random LAN (Laboratory Accessioning Number), and completing data entry that the electronic chain of custody system lacks.

Screening is an initial check for drug presence. While cost-effective for ruling out drug use in most samples, a positive result needs confirming for legal purposes. Presumptive positive samples require additional confirmation.

If a sample is initially positive, additional hair is extracted for further analysis. In Extraction, drugs are isolated from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), making hair testing more complex.

Any screening positives are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positives undergo washing before confirmation if necessary. The complete process from Accessioning to Confirmation adheres to CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
