What it is, How it works

Hair examinations have emerged as an influential method for identifying usage of drugs and alcohol. Hair captures a long-term record of alcohol and other drugs by embedding biomarkers within the strands of the hair as it grows. When obtained near the scalp, hair can provide up to roughly a 3-month detection timeframe for alcohol and drugs. Hair is straightforward to gather, moderately tough to tamper with, and simple to transport.

A 1.5-inch segment of roughly 200 hair strands (similar in thickness to a #2 pencil) closest to the scalp will yield 100mg of hair, which is the optimal sample size for analysis and validation. For EtG, add-ons, and/or examinations exceeding 10 panels, 150mg of the sample is advised. It is advisable to measure the specimen on a jeweler’s scale. Should scalp hair be unavailable, a comparable amount of body hair might be collected. When mentioning head hair, we specifically refer to scalp hair. Body hair pertains to all other hair varieties (facial, axillary, etc.).

Process Outline

The key steps in the laboratory mechanism of producing a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary handling of a sample into a laboratory’s database. This involves ensuring that the sample was correctly sealed and transported, allocating a random LAN (Laboratory Accessioning Number), and finalizing any further data entry absent from an electronic custody tracking system.

Screening is an initial rapid inspection for drug misuse. Although Screening is a cost-effective method to exclude drug consumption in a majority of samples, a positive screening result must be validated to stand in court. Any samples that receive a presumptive positive result in Screening do need a subsequent verification.

If a sample is presumptively positive during the Screening phase, additional hair is extracted from the initial sample and prepped for Extraction. During this step, drugs are extracted from hair at considerably lower concentrations than other techniques (e.g., urine or oral fluid), which makes hair drug testing one of the most challenging techniques to execute.

The Confirmation of any positive result from screenings is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as needed. The complete laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
