What it is, How it works

Collected near the scalp, hair samples can offer a detection window of up to about 90 days for alcohol and other drugs. It is easy to collect hair, challenging to tamper with, and simple to ship.

A 1.5-inch segment comprising approximately 200 hair strands (roughly equivalent to a #2 pencil's diameter) collected closest to the scalp will produce 100mg, the preferred quantity for both screening and confirmation processes. For EtG, supplementary tests, and/or analyses exceeding 10 panels, a 150mg sample is advisable. Using a jeweler's scale to weigh the sample is recommended. If scalp hair isn't an option, a comparable amount of body hair can be used instead. When mentioning head hair, it refers exclusively to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The four core phases in the laboratory processing of drug testing results include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial handling phase where the sample is integrated into the laboratory system. This step ensures the sample was sealed and transported correctly, assigns a unique LAN (Laboratory Accessioning Number), and completes any remaining data input absent from an electronic chain of custody setup.

Screening is an initial rapid assessment for drugs of abuse. While it is an economical strategy to dismiss drug presence in most samples, any positive screen requires confirmation for legal use. Samples that test presumptively positive in Screening necessitate further confirmation.

If a sample is indicative of a positive result at the Screening phase, additional hair is extracted from the initial collection and prepped for Extraction. Here, drugs are isolated from hair in lower concentrations than in other methods (e.g., urine or oral fluids), illustrating why hair drug screening is a particularly complex technique.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that show presumptive positivity are cleansed as necessary before confirmation. The entire laboratory process, spanning from Accessioning to Confirmation, undergoes scrutiny under CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
