A sample measuring 1.5 inches, comprising about 200 strands (approximately the size of a #2 pencil) nearest the scalp, yields 100mg, ideal for both screening and validation. For EtG, additional panels over ten, 150mg is suggested. We advise using a jeweler's scale to weigh the sample. In the absence of scalp hair, an equivalent quantity of body hair can be collected. References to head hair pertain solely to scalp hair, while body hair includes other types (facial, axillary, etc.).

Process Overview

The procedure for processing a laboratory drug test encompasses Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the starting point, where a sample is entered into the lab's system. This step checks that the sample is securely sealed and shipped, allocates a LAN (Laboratory Accessioning Number), and completes data entry not covered by an electronic chain of custody process.

Screening is an initial test for illicit substances. While it effectively excludes drug use for most samples, a positive result must be confirmed to be legally acceptable. Samples exhibiting presumptive positivity in Screening require further confirmation.

Should the Screening process indicate presumptive drug presence, a portion of the initial hair sample is re-prepared for Extraction. This phase involves retrieving substances from hair at lower concentrations than other methods (like urine or saliva), making it a challenging but precise technique.

A positive Screening outcome is confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, positive samples are washed as required. From Accessioning to Confirmation, the entire lab process is reviewed following both CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
