For EtG, additional tests, and/or panels exceeding 10, 150mg of the specimen is advisable. We recommend using a jeweler’s scale for measuring the specimen. If scalp hair isn't available, an equivalent amount of body hair can be utilized. "Head hair" refers specifically to scalp hair. "Body hair" encompasses all other hair varieties (e.g., facial, axillary, etc.).

Process Overview

The key phases in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial handling of a sample within a laboratory’s framework. This entails verifying that the sample was sealed and dispatched correctly, allocating a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data entry not accommodated by an electronic chain of custody system.

Screening involves a preliminary rapid check for drugs of abuse. Although Screening is economical for excluding drug use in most samples, a positive screen requires confirmation for legal acceptability. Samples that show presumptive positives in Screening necessitate a subsequent confirmation.

If a sample shows presumptive positivity in the Screening phase, more hair is extracted from the initial sample and processed for Extraction. In this phase, drugs are isolated from hair at a much lower concentration than through other methods (e.g., urine or saliva), which is why hair drug testing is the most challenging method to execute.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing before confirmation as necessary. The entire lab process from Accessioning to Confirmation is inspected under both the CAP (College of American Pathologists) Hair categorization and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
