When collected near the scalp, hair can offer up to a 3-month detection period for alcohol and drugs. It is easy to collect, relatively hard to tamper with, and simple to transport.

A 1.5-inch sample comprising roughly 200 hair strands (approximately the width of a #2 pencil) taken nearest to the scalp will yield about 100mg of hair, which is the optimal amount for examination and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the sample is suggested. It is advisable to weigh the sample using a jeweler’s scale. If scalp hair cannot be provided, a similar quantity of body hair may be used. When referencing head hair, it refers specifically to scalp hair. Body hair includes all other hair forms (facial, axillary, etc.).

Process Overview

There are four key steps in the laboratory handling of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary stage of integrating a sample into the laboratory’s system. This step ensures the sample was sealed and shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any manual data entry not facilitated by an electronic chain of custody system.

Screening is a preliminary procedure for spotting drugs of abuse. Although Screening is an economical approach to dismiss drug use in most samples, any positive screening must be verified to be legally acceptable. Samples with presumed positive findings in Screening necessitate a further confirmation step.

Should a sample test positive during the Screening phase, additional hair is taken from the original sample and prepared for Extraction. During this phase, drugs are separated from hair at a much lower concentration compared to other methodologies (e.g., urine or oral fluid), which is why hair drug testing presents more challenges.

Any positive Screening results undergo Confirmation using techniques such as GC/MS, GC/MS/MS, or LC/MS/MS. All positive samples receive a wash as needed before Confirmation. The complete laboratory procedure from Accessioning to Confirmation is audited according to both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 accreditation standards.
