What it is, How it works

Hair testing has emerged as a prominent instrument in detecting drug and alcohol consumption. Hair offers a prolonged account of alcohol and other substances by retaining biomarkers within the fibers of the developing hair strand. If collected near the scalp, hair can present approximately a three-month detection timeframe for alcohol and other substances. Collection is straightforward, moderately resistant to tampering, and convenient to ship.

A 1.5-inch specimen of about 200 hair strands (comparable to the thickness of a #2 pencil) nearest the scalp supplies 100mg of hair, the optimal amount for both screening and confirmation. For EtG, additional tests, and/or analyses exceeding 10 panels, 150mg of the material is suggested. It is advisable to measure the material on a jeweler’s scale. Should scalp hair be unobtainable, an equivalent quantity of body hair can be gathered. "Head hair" strictly pertains to scalp hair, while "Body hair" covers all other hair varieties (e.g., facial, axillary).

Process Overview

There are four primary phases in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a laboratory’s administration system. This step verifies that the sample was properly sealed and shipped, allocates a random LAN (Laboratory Accessioning Number), and finalizes any remaining data entry not supplied by an electronic chain of custody system.

Screening consists of an initial rapid assessment for substances of abuse. While Screening offers a cost-efficient method to negate drug use in most samples, a positive screen necessitates confirmation for legal admissibility. Any samples that indicate a presumptive positive in Screening demand subsequent confirmation.

If a sample is presumed positive during the Screening phase, additional hair is extracted from the original specimen and prepared for Extraction. During this phase, drugs are extracted from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), which is why hair drug screening is notoriously challenging to execute.

Confirmation of any positive screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Every presumptive positive sample is washed prior to confirmation as needed. The complete laboratory procedure from Accessioning to Confirmation is scrutinized under both CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The pitfalls encountered when the exogenous/endogenous nature of a substance is questioned were presented and discussed. The special issue presents also manuscripts that delve into the ethical implications of drug testing in drug facilitated sexual assaults.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
