Process Overview

The lab processing of a drug test result involves four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the primary processing of a sample into a lab's system, which includes confirming the sample's proper sealing and delivery, assigning a random LAN (Laboratory Accessioning Number), and completing data entry that's not supplied by an electronic chain of custody system.

Screening is an initial evaluation for controlled substance presence. While Screening is a cost-efficient means of negating drug use in most samples, a positive result must be confirmed to be legally valid. Any samples presumed positive during Screening require subsequent confirmation.

If a sample appears positive during Screening, additional hair is taken from the original sample and readied for Extraction. During this phase, drugs are extracted from hair at lower concentrations than other methods (like urine or oral fluid), highlighting the challenges of hair drug screening.

Confirmation of any positive result from Screening is undertaken using GC/MS, GC/MS/MS, or LC/MS/MS technology. All samples presumed positive are washed prior to confirmation, as deemed necessary. The full laboratory procedure from Accessioning through Confirmation undergoes review based on both the CAP (College of American Pathologists) Hair standard and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
