What it is, How it works

Hair analysis has emerged as an effective instrument for identifying drug and alcohol consumption. Hair captures a prolonged history of alcohol and other substances by embedding biomarkers in the strands' fibers. When sourced near the scalp, it can offer an approximately three-month detection interval for alcohol and other drugs. Hair collection is straightforward, somewhat resistant to tampering, and simple to transport.

A sample spanning 1.5 inches, consisting of around 200 hair strands (roughly the size of a #2 pencil) nearest the scalp, yields 100mg of hair, which is ideal for both screening and confirmation. For EtG, additional tests, and/or beyond 10-panel tests, 150mg of the sample is preferable. It's advised to measure the specimen using a jeweler’s scale. If scalp hair isn't available, a similar quantity of body hair can be collected. In the context of head hair, we specifically mean scalp hair. Body hair includes all other varieties (facial, axillary, etc.).

Overview of the Procedure

The primary steps in processing drug test results in a lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails processing a sample into the lab's system initially. This process involves verifying proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing data entries that aren't covered by an electronic chain of custody system.

Screening involves an initial rapid assessment for drugs of abuse. While it's cost-effective for ruling out drug use in most samples, a positive screening needs confirmation to be admissible in court. Any samples that show a presumptive positive in Screening require secondary confirmation.

If a sample shows a presumptive positive during Screening, more hair is extracted from the initial sample for preparation in the Extraction phase. During this stage, drugs are extracted from hair at a much lower concentration than in other methods (such as urine or oral fluid), highlighting why hair drug screening is the most challenging method to execute.

Confirmation of any positive screening results occurs through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are cleaned as necessary before confirmation. The complete lab process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation as well as accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
