What it is, How it works

Hair testing is now recognized as a significant technique for detecting drug and alcohol consumption. For EtG and additional tests exceeding 10 panels, 150mg of the sample is advised. It is recommended to weigh the sample using a jeweler’s scale. In the absence of scalp hair, a similar quantity of body hair may be used. References to head hair pertain solely to scalp hair, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

There are four primary steps in processing a drug test result in the lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a sample into a laboratory’s system. This process verifies proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and fulfills any necessary data entry not covered by an electronic chain of custody system.

Screening offers an initial rapid assessment for drug abuse. Although Screening is a cost-efficient method for ruling out drug use in most samples, a positive screen must be verified to be legally acceptable in court. Samples that are presumptively positive in Screening will need a secondary confirmation.

If a sample is presumptively positive in Screening, additional hair is extracted from the initial sample and readied for Extraction. During this phase, drugs are isolated from hair at significantly lower concentrations compared to other methodologies (e.g., urine or oral fluid), rendering hair drug screening the most complex procedure to execute.

Confirmation of any positive screen is done using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo a pre-confirmation wash as necessary. The entire lab process from Accessioning through Confirmation is conducted according to the CAP (College of American Pathologists) Hair program and complies with ISO / IEC 17025 standards.
