What it is, How it works

The analysis of hair samples is recognized as a significant method for identifying drug and alcohol usage. Hair captures long-term evidence of alcohol and drug consumption by embedding biomarkers within its strands. When taken near the scalp, hair can offer an approximate 3-month timeframe for drug and alcohol detection. Collection is straightforward, tampering is challenging, and shipping is convenient.

To obtain 100mg of hair—ideal for both screening and confirmatory purposes—a 1.5-inch segment consisting of roughly 200 strands (equivalent to a #2 pencil’s thickness) near the scalp is required. For EtG, supplementary assessments, and/or panels exceeding 10, a specimen weight of 150mg is preferred. Using a jeweler’s scale for measurement is advisable. If scalp hair is unreachable, an equivalent quantity of body hair can be used. Mention of head hair pertains solely to scalp hair, whereas body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The lab processing of a drug test involves four principal steps: Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, the sample goes through the initial lab system processing. This involves ensuring the sample’s seal and shipment are intact, assigning a LAN (Laboratory Accessioning Number) randomly, and completing any additional information entry absent in an electronic custody system.

Screening serves as a preliminary, rapid assessment for drug presence. Though cost-effective for eliminating drug use in most samples, positive screens require confirmation for court proceedings. Presumptively positive samples in Screening necessitate a secondary confirmation.

For samples presumptively positive during Screening, additional hair from the original sample is prepped for Extraction. This step involves removing drugs from hair at lower concentrations compared to other methods (e.g., urine or oral fluids), underscoring the complexity of hair drug screening.

Confirmation of positive screen results is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Samples showing presumptive positivity undergo a pre-confirmation wash as necessary. The entire sequence from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair directive and ISO / IEC 17025 accreditation guidelines.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
