A hair sample of 1.5 inches, consisting of about 200 strands (similar to the girth of a #2 pencil) nearest to the scalp, provides 100mg of hair, which is optimal for screening and confirmation purposes. For EtG, additional tests, and/or tests exceeding 10 panels, it is advisable to have 150mg of the sample. Utilizing a jeweler’s scale to weigh the sample is advised. If there is no scalp hair available, a comparable amount of body hair can be used. When we mention head hair, it pertains solely to scalp hair. Body hair includes all other hair types (such as facial, underarm, etc.).

Process Overview

The laboratory processing for a drug test result incorporates four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial incorporation of a sample into a lab’s system. This entails confirming that the sample was properly sealed and shipped, allocating a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not covered by an electronic chain of custody system.

Screening consists of an initial rapid inspection for drugs of abuse. While Screening is an economical method to exclude drug usage in most samples, a positive screen necessitates confirmation to be valid in a legal setting. Any samples preliminarily positive during Screening require a secondary confirmation.

If a sample is provisionally positive at the Screening stage, additional hair is taken from the original sample and readied for Extraction. In this phase, drugs are extracted from the hair at a considerably lower concentration than other methods (e.g., urine or oral fluid), making hair drug screening the most complex method to execute.

Confirmation of any positive screening result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are provisionally positive undergo washing before confirmation as required. The entire lab process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
