What it is, How it works

When taken near the scalp, hair samples can offer nearly a 3-month window for detecting alcohol and drugs. Hair sampling is straightforward, less prone to tampering, and easy to transport.

A hair sample of 1.5 inches consisting of roughly 200 strands (comparable to the width of a #2 pencil) from the scalp will yield 100mg of hair, serving as the perfect amount for both screening and confirmation. For EtG, extra tests, and/or more extensive panel tests, 150mg of specimen is advisable. We advise measuring the specimen using a jeweler's scale. If scalp hair is not accessible, an equivalent amount of body hair is acceptable. When referencing head hair, we specifically mean scalp hair. Body hair includes all other hair forms (facial, axillary, etc.).

Process Overview

There are four crucial steps in processing drug test results in a lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial inclusion of a sample into the lab's system, encompassing verification that it was suitably sealed and shipped, allocation of a unique LAN (Laboratory Accessioning Number), and any supplementary data entry absent from the electronic chain of custody system.

Screening provides a rapid preliminary assessment for drug misuse. While Screening offers a budget-friendly means to exclude drug use in most cases, confirmed positive results are needed for legal validity. Any initially positive findings in Screening must undergo further confirmation.

If a sample shows a presumptive positive during Screening, additional hair is extracted from the original sample and prepped for Extraction. In this step, drugs are exuded from hair at notably lower concentrations than other methods (e.g., urine or oral fluid), making hair analysis a more challenging approach.

Confirmation of any positive findings from Screening is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All preliminary positive samples undergo washing before confirmation if required. The full laboratory sequence, from Accessioning to Confirmation, is assessed following both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
