What it is, How it works

Hair analysis is regarded as a potent means for identifying drug and alcohol consumption. Hair retains an extended history of substance use by enclosing biomarkers within the growing strand's fibers. When taken near the scalp, hair can offer up to a 3-month timeframe for detecting alcohol and other substances. It is straightforward to collect, somewhat resistant to tampering, and simple to transport.

A sample measuring 1.5 inches, consisting of about 200 strands (approximately the thickness of a #2 pencil) closest to the scalp, will provide 100mg of hair, which is optimal for screening and confirmation. For EtG, additional requests, and/or tests exceeding 10 panels, a collection of 150mg is advised. We advise utilizing a jeweler's scale to measure the sample's weight. If hair from the scalp is inaccessible, a comparable quantity of body hair can be collected. References to head hair imply only scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into the laboratory's system, which involves ensuring the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and finalizing any missing data not available through an electronic chain of custody.

Screening involves a preliminary rapid assessment for drugs of abuse. Although Screening is an economical method to exclude drug usage in most samples, a positive result requires confirmation for legal admissibility. Any samples assumed positive in Screening must undergo secondary confirmation.

If a sample is assumed positive in Screening, additional hair is taken from the original specimen for Extraction. During Extraction, drugs are retrieved from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), which is why hair drug screening presents a greater challenge.

Confirmation of any positive Screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All assumed positive samples undergo washing before confirmation if necessary. The entire laboratory process from Accessioning to Confirmation undergoes review under the CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
