What it is, How it works

When obtained near the scalp, hair samples can reveal up to approximately three months of consumption data for alcohol and other substances. These samples are easy to collect, have a low risk of tampering, and are straightforward to transport.

A sample size of 1.5 inches, equating to around 200 hair strands (equivalent to the thickness of a #2 pencil) nearest to the scalp, yields about 100mg of hair, which is ideal for both screening and confirmation. For tests such as EtG, additional tests, and/or those exceeding 10 panels, a sample of 150mg is advisable. We suggest using a jeweler’s scale for weighing the specimen. In cases where scalp hair cannot be collected, a comparable quantity of body hair should be gathered. When we mention head hair, it refers strictly to scalp hair. Body hair encompasses all other types of hair (such as facial and axillary hair).

Process Overview

There are four primary stages in laboratory processing of drug test results: Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, the sample is initially processed into the lab's system. This step ensures the sample was properly sealed and sent, assigns a random LAN (Laboratory Accessioning Number), and completes any data entry missing from an electronic chain of custody system.

Screening offers a quick preliminary check for drugs of abuse. Though it's a cost-efficient way to dismiss drug use in most samples, a positive screen demands further confirmation for legal admissibility. Presumptively positive samples in Screening necessitate secondary validation.

Confirmation of any positive screening result occurs via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing before confirmation as required. The complete laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair classification and ISO / IEC 17025 accreditation standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
