A segment measuring 1.5 inches, comprising approximately 200 hair strands (roughly the girth of a #2 pencil) taken closest to the scalp, yields 100mg of hair—the ideal quantity for preliminary tests and confirmation. For EtG, additional tests, or panels exceeding 10, a specimen of 150mg is advised. We suggest measuring the specimen's weight on a jeweler’s scale. If scalp hair is insufficient, a comparable quantity of body hair may be procured. “Head hair” refers specifically to scalp hair, while “body hair” encompasses other types (such as facial or underarm hair).

Process Overview

There are four principal stages in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the sample’s initial incorporation into the laboratory system. This process includes ensuring the sample's secure sealing and transport, issuing a unique LAN (Laboratory Accessioning Number), and finalizing any remaining paperwork not handled by an electronic chain of custody system.

Screening provides a preliminary assessment for drug presence. While cost-efficient, screening primarily disqualifies drug use in most cases, but a positive outcome needs verification for legal validity. Any Screening results suggesting drug presence require confirmatory testing.

If a sample yields a potential positive result during Screening, additional hair is collected from the initial sample for the Extraction process. Drugs are extracted at considerably lower concentrations from hair than from other samples (e.g., urine or oral fluids), hence, hair drug analysis is among the most complex testing techniques.

Positive Screening results undergo confirmation using GC/MS, GC/MS/MS, or LC/MS/MS technologies. All samples indicated as positive are appropriately cleansed before confirmation. The entire lab procedure, from Accessioning to Confirmation, is evaluated according to the CAP (College of American Pathologists) Hair guidelines and ISO/IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
