When collected near the scalp, it can offer a detection period of up to about three months for alcohol and drugs. Hair is straightforward to collect, relatively resistant to tampering, and convenient for transport.

A 1.5-inch segment consisting of roughly 200 hair strands (comparable to the circumference of a #2 pencil) from close to the scalp provides 100mg of hair, the optimal amount for both screening and confirmation. For EtG add-ons or tests exceeding 10 panels, a 150mg sample is advised. It's beneficial to use a jeweler’s scale for weighing the sample. If scalp hair is not present, an equivalent quantity of body hair can be collected. When we refer to head hair, we mean scalp hair specifically. Body hair includes other hair types such as facial, axillary, and similar.

Process Overview

The crucial phases entailed in processing a drug test in a lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the primary processing of a sample into a laboratory's system. This stage verifies the sample has been sealed and shipped correctly, a random LAN (Laboratory Accessioning Number) is assigned, and any further data entry not covered by an electronic chain of custody system is finalized.

Screening denotes an initial swift examination for drug abuse. Although this is a cost-efficient technique to dismiss drug usage in most samples, any positive screen needs confirmation for legal admissibility. Samples that show presumptive positives in Screening do require additional confirmation.

If a sample tests presumptive positive during the Screening process, additional hair is taken from the initial sample and readied for Extraction. Here, drugs are extracted from hair at lesser concentrations than those found in other methods (such as urine or oral fluid), making hair drug screening a more challenging technique to implement.

Confirming any positive screening results is done using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are pre-washed as necessary before confirmation. The whole laboratory sequence from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 norms.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
