What it is, How it works

When hair is collected from near the scalp, it can show evidence of alcohol and drug use for up to about three months. Hair is easy to collect, hard to tamper with, and straightforward to transport.

A 1.5-inch segment of approximately 200 hair strands (around the diameter of a #2 pencil) nearest to the scalp provides 100mg of hair, which is optimal for screening and confirmation tests. For EtG, additional tests, or those exceeding 10 panels, a sample of 150mg is suggested. It is advisable to use a jeweler’s scale to weigh the sample. If scalp hair is not an option, an equivalent amount of body hair can be gathered. When head hair is mentioned, it specifically refers to scalp hair, while body hair denotes all other types (facial, axillary, etc.).

Process Overview

The laboratory process for drug testing involves four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with integrating a sample into a laboratory’s system upon receipt. This step ensures that the sample is sealed and shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any missing data not provided by an electronic chain of custody system.

Screening performs an initial examination for drugs of abuse. While it's a cost-efficient method to rule out drug use in most samples, results showing positive require confirmation to be court-admissible. Presumptively positive samples in Screening necessitate a secondary confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the same specimen for further preparation in the Extraction phase. Confirmation of any positive screening is done via GC/MS, GC/MS/MS, or LC/MS/MS. Any samples presumptively positive are washed before confirmation as necessary. The full process from Accessioning to Confirmation is checked according to both the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 accreditation standards.
