A sample of 1.5 inches, consisting of roughly 200 hair strands (approximately the size of a #2 pencil) closest to the scalp, yields 100mg of hair, which is the ideal amount for both screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, it is advised to obtain 150mg of the specimen. Measuring the specimen using a jeweler’s scale is suggested. In the absence of scalp hair, an equivalent quantity of body hair can be taken. The term "head hair" specifically denotes scalp hair only. "Body hair" encompasses all other types of hair (such as facial, axillary, etc.).

Process Overview

The key phases involved in laboratory processing for a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into the laboratory’s framework. This involves ensuring the sample was properly sealed and transported, allotting a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent in an electronic chain of custody system.

Screening is about conducting a preliminary swift evaluation for drug abuse. Although Screening is an economical way to exclude drug usage in the majority of samples, a positive screen requires confirmation to be valid in legal settings. Samples that show presumptively positive results in Screening necessitate secondary confirmation.

In instances where a sample indicates presumptively positive in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. During this phase, drugs are extracted from hair at considerably lower concentrations than in other procedures (e.g., urine or oral fluid), which makes hair drug screening a highly challenging procedure.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptive positive undergo washing before confirmation as required. The entire laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
