A 1.5-inch section with about 200 hair strands (around the width of a #2 pencil) taken nearest to the scalp will yield 100mg of hair, which is the optimal sample amount for both screening and confirmation. For EtG, additional tests, and/or tests beyond 10 panels, a sample weight of 150mg is suggested. It is advisable to weigh the sample using a jeweler’s scale. If head hair isn't accessible, a similar amount of body hair can be utilized. References to head hair mean scalp hair exclusively. Body hair encompasses all other hair types (such as facial, armpit hair, etc.).

Process Overview

The laboratory processing of a drug test result includes these four steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial procedure that registers a sample in the lab's system. This procedure includes checking if the sample was correctly sealed and sent, assigning it a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody system.

Screening constitutes a preliminary inspection for drug abuse. Though Screening is a budget-friendly method to eliminate drug usage in most samples, a positive screen necessitates confirmation to be valid in a legal context. Samples with presumptive positive results in Screening require further confirmation.

Should a sample test presumptively positive in Screening, more hair from the specimen is extracted and set for the Extraction stage. During Extraction, drugs are retrieved from hair at much lower concentrations than with other methods (such as urine or oral swab), which makes hair drug testing a challenging method to execute.

The Confirmation of any positive Screening outcome is done via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples undergo washing before confirmation as required. From Accessioning to Confirmation, the entire lab process is examined under CAP (College of American Pathologists) Hair designation and certified by ISO / IEC 17025 standards.
