If scalp hair is unavailable, a similar quantity of body hair can be gathered. When discussing head hair, it refers exclusively to scalp hair. Body hair denotes all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four main phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial handling of a sample within a lab’s framework. This comprises verifying the sample’s proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not furnished by an electronic chain of custody system.

Screening provides an initial rapid assessment for substance abuse. While Screening is a cost-efficient method to discount drug usage for most samples, a positive screen must be validated for legal admissibility. Any samples with a presumptively positive Screening require a further confirmation.

If Screening suggests a positive result, more hair is taken from the original specimen for Extraction preparation. During this step, drugs are extracted from hair at a much lower concentration than in other techniques (e.g., urine or oral fluid), illustrating the complexity of hair drug screening procedures.

Confirmation of any positive screening outcome is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. Every presumptive positive sample is washed prior to confirmation if necessary. The comprehensive laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and adherence to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
