What it is, How it works

Hair strands capture biomarkers within their fibers, offering a prolonged overview of alcohol and drug use. When harvested near the scalp, hair can reveal usage patterns over a timeframe of up to around 3 months. Hair collection is straightforward, challenging to falsify, and simple to transport.

Gathering a 1.5-inch segment consisting of approximately 200 hair strands (comparable to the diameter of a #2 pencil) nearest to the scalp produces 100mg of hair, which is the ideal quantity for screening and confirmation processes. For EtG or additional tests exceeding 10 panels, 150mg of the sample is advisable. We propose weighing the sample using a jeweler’s scale. Should scalp hair be inaccessible, a similar amount of body hair may be utilized. References to head hair pertain solely to scalp hair. Other hair classifications (like facial or axillary) are designated as body hair.

Procedure Synopsis

There are four principal stages in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of a sample into the lab’s framework. This involves ensuring the sample is properly sealed and transported, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any necessary data input not covered by an electronic chain of custody system.

Screening involves an initial cursory examination for abused substances. Although it offers a cost-efficient strategy to initially rule out drug presence for most samples, a positive result necessitates confirmation for legal admissibility. Presumptive positives from Screening require supplementary verification.

Upon a presumptively positive Screening result, further hair is extracted from the initial sample and prepared for Extraction. At this point, substances are drawn from hair at a significantly lower concentration compared to alternative methods (like urine or oral fluid), making hair drug testing the most challenging methodology to execute.

Confirmation of any positive screening outcome is undertaken using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. Every step from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
