What it is, How it works

When taken near the scalp, hair can indicate up to a roughly 3-month range for detecting alcohol and other drugs. Hair is easy to collect, relatively hard to tamper with, and straightforward to transport.

A 1.5-inch section of around 200 strands of hair (roughly the thickness of a #2 pencil) nearest the scalp will yield 100mg of hair, which is the optimal amount for both screening and confirmation tests. For EtG analysis, additional testing, and/or tests beyond 10 panels, 150mg of the hair sample is advised. It is suggested to weigh the specimen using a jeweler’s scale. Should scalp hair be inaccessible, a corresponding amount of body hair can be used. When referring to head hair, it specifically means scalp hair. Body hair pertains to all other hair forms (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the lab’s framework. This encompasses confirming the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent in an electronic chain of custody system.

Screening conducts an initial rapid assessment for drug usage. Screening is an economical means to rule out drug consumption in the majority of samples, but a positive screen must be confirmed to be acceptable in legal proceedings. Any samples that show a presumptive positive in Screening require further confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the initial sample and readied for Extraction. Here, drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening results is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The complete laboratory process from Accessioning to Confirmation is subjected to review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
