For EtG, additional tests, and/or tests exceeding 10 panels, a 150mg sample is advised. It is recommended to weigh the sample with a jeweler’s scale. Should scalp hair be unavailable, an equivalent portion of body hair may be utilized. Head hair specifically refers to scalp hair only, whereas body hair includes all other types (such as facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test outcome comprises four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory’s system. This involves confirming that the sample was correctly sealed and shipped, designating a random LAN (Laboratory Accessioning Number), and completing any outstanding data entry not provided by an electronic chain of custody system.

Screening involves a preliminary, swift assessment for substances of abuse. While Screening is a cost-efficient approach to discount drug use in most samples, a positive screen requires confirmation for court admissibility. Any samples that yield presumptive positives in Screening necessitate a further confirmation.

If a sample indicates a presumptive positive in the Screening phase, additional hair is extracted from the initial sample for Extraction. This phase involves drugs being extracted from hair at a significantly lower concentration compared to other methods (e.g., urine or oral fluid), thereby making hair drug screening the most challenging method to execute.

Confirmation for any positive Screening result is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives undergo a wash before confirmation as necessary. The complete laboratory sequence from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
