What it is, How it works

Hair testing is recognized as an effective method for detecting drug and alcohol consumption. Hair retains a long-term trace of alcohol and drugs by capturing biomarkers in the hair's fibers as it grows. When collected near the scalp, hair offers a detection window of up to roughly 3 months for alcohol and other drugs. Hair collection is straightforward, relatively tamper-resistant, and easily shipped.

A sample of 1.5 inches consisting of about 200 hair strands (roughly the thickness of a #2 pencil) near the scalp yields 100mg of hair, optimal for screening and confirmation. For EtG, additional tests, and/or those exceeding 10 panels, a specimen of 150mg is advisable. We recommend using a jeweler's scale for weighing. If scalp hair isn't available, a comparable amount of body hair can be collected. Our reference to head hair pertains solely to scalp hair. The term body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial sample processing into a lab's system. It involves verifying proper sealing and shipping of the sample, allocating a random LAN (Laboratory Accessioning Number), and completing any additional data entry not handled by an electronic chain of custody system.

Screening is an initial swift check for drug abuse. Screening is cost-effective for ruling out drug use in most samples; however, a positive screen must be confirmed for court admissibility. Samples flagged as presumptive positives during Screening require secondary confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the original sample for Extraction. This stage involves extracting drugs from hair at concentrations lower than in other methods (e.g., urine or oral fluid), contributing to hair drug screening's complexity.

Confirmation of any positive screening outcome is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumed positive samples are cleaned before confirmation when necessary. The complete lab process, from Accessioning to Confirmation, undergoes review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
