A sample measuring 1.5 inches and composed of about 200 hairs (roughly the diameter of a #2 pencil) close to the scalp provides 100mg of hair, the optimal sample size for both initial and comprehensive tests. For tests such as EtG, additional panels, and exams exceeding 10 panels, a sample size of 150mg is advisable. Utilizing a jeweler’s scale to weigh the specimen is recommended. If scalp hair is uncollectible, a corresponding amount of body hair can be utilized. When discussing head hair, we specifically mean scalp hair. Body hair pertains to all other forms (facial, axillary, etc.).

Process Overview

There are four primary phases in processing a drug test in a laboratory: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where the sample is integrated into the lab’s systems. This step ensures the sample was correctly sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and involves completing any supplementary data input absent in an electronic chain of custody system.

Screening serves as a preliminary assessment for illicit drug presence. While it provides a cost-efficient method to exclude drug usage in most cases, any positive finding must be corroborated for legal acceptability. Samples showing preliminary positive results in Screening necessitate further verification.

If a sample tests preliminarily positive at the Screening phase, additional portions of hair from the existing sample are selected for Extraction. In this phase, drugs are isolated from hair at much lower levels compared to other methods (e.g., urine or saliva), thus making hair drug testing the most challenging methodology.

Verification of any positive screening outcomes is achieved using GC/MS, GC/MS/MS or LC/MS/MS technologies. All samples that are preliminarily positive undergo cleansing prior to confirmatory testing as needed. The entire laboratory workflow from Accessioning to Confirmation is conducted under the CAP (College of American Pathologists) Hair criteria and complies with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
