The four core steps in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial intake of a sample into a laboratory system. This incorporates verifying the sample's seal and proper shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not furnished by an electronic chain of custody system.

Screening involves a preliminary rapid check for substance abuse. Although Screening is a cost-efficient method to exclude drug usage for most samples, a positive screen must be corroborated to be valid in legal proceedings. Any samples deemed presumptively positive in Screening necessitate a subsequent confirmation.

If a sample appears presumptively positive during Screening, additional hair is drawn from the initial sample and prepped for Extraction. At this phase, drugs are extracted from hair at a significantly lower concentration than in other methods (ex. urine or oral fluid), which is why hair drug testing is the most challenging method to conduct.

Confirmation of any positive screening finding is carried out using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as required. The entire lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
