In cases where scalp hair is not available, a similar quantity of body hair can be used. The term "head hair" specifically refers to scalp hair, whereas "body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The drug testing process in the laboratory consists of four major phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial procedure of integrating a sample into a laboratory’s system. This step checks that the sample was properly sealed and shipped, assigns an arbitrary LAN (Laboratory Accessioning Number), and finalizes any additional data entry not available through an electronic chain of custody.

Screening serves as a preliminary evaluation for drugs of misuse. While Screening offers a cost-efficient means to exclude drug use in most specimens, a positive screen must be confirmed for it to be legally viable. Samples that yield a presumptive positive in Screening necessitate a secondary verification.

For samples that test presumptively positive during the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. In this phase, drugs are extracted from hair at lower concentrations compared to alternative methods (e.g., urine or oral fluid), which makes hair drug screening a challenging method to execute.

The Confirmation of any positive screen is completed using GC/MS, GC/MS/MS, or LC/MS/MS methods. Samples with presumptive positive results are cleansed before Confirmation as required. The entire process from Accessioning through Confirmation is reviewed under the CAP (College of American Pathologists) Hair designation and under the accreditation of ISO / IEC 17025 standards.
