In the absence of scalp hair, an equivalent volume of body hair is acceptable. The term “head hair” specifically denotes scalp hair. In contrast, “body hair” encompasses facial, axillary, and other types.

Process Overview

The laboratory processing of drug test outcomes comprises four essential phases: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into the laboratory’s system, ensuring it was properly sealed and shipped, assigning a random Laboratory Accessioning Number (LAN), and completing any additional data entry not captured electronically.

Screening offers a preliminary assessment for drug abuse. This cost-effective method can eliminate drug presence in most cases; however, a positive result necessitates confirmation for legal admissibility. Any sample testing positive during Screening requires further confirmation.

Should a sample test positive in Screening, additional hair from the original sample is used for Extraction. Here, drugs are extracted at much lower concentrations than other techniques (e.g., urine or oral fluids), making hair testing particularly challenging.

Confirmation of positive results is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS. All positive samples are cleansed before Confirmation as needed. The entire Accessioning to Confirmation process undergoes review by the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
