For EtG, additional tests, and/or those exceeding 10 panels, a specimen of 150mg is advised. Using a jeweler’s scale to weigh the specimen is recommended. If scalp hair is not available, a similar quantity of body hair can be collected. References to head hair pertain exclusively to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary phases: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes initial integration into the laboratory's system. This step ensures the sample was properly sealed and transported, assigns it a random LAN (Laboratory Accessioning Number), and completes any necessary data input not covered by an electronic chain of custody system.

Screening entails a rapid preliminary evaluation for illicit drugs. While Screening is a cost-efficient method for eliminating drug usage in most samples, a positive result necessitates confirmation for legal admissibility. Any samples with a presumptive positive in Screening require further confirmation.

For samples with a presumptive positive during Screening, additional hair is extracted from the original specimen for Extraction. During Extraction, drugs are withdrawn from hair at significantly lower concentrations than those in alternative techniques (e.g., urine or oral fluid), which contributes to why hair drug screening is the most challenging approach to execute.

GC/MS, GC/MS/MS, or LC/MS/MS methods are employed to confirm any positive Screening outcomes. All samples classified as presumptive positive undergo cleansing, as necessary, before confirmation. The complete laboratory process, from Accessioning to Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
