What it is, How it works

Hair serves as a historical record, trapping biomarkers within the fibers of the hair strand as it grows. When collected near the scalp, hair can reveal drug or alcohol use over an approximate 3-month period. Hair is straightforward to collect, relatively tamper-proof, and easy to transport.

A 1.5-inch section, consisting of about 200 hair strands (similar in size to a #2 pencil) closest to the scalp, equates to 100mg of hair, which is an optimal sample for both screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, a specimen of 150mg is suggested. We advise using a jeweler’s scale to weigh the specimen. If scalp hair is not available, a similar quantity of body hair may be used. When mentioning head hair, it specifically refers to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result comprises four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary processing of a sample into a lab’s system. This step verifies that the sample was sealed and transported correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry that is not provided by an electronic chain of custody system.

Screening consists of a preliminary quick check for abused drugs. While Screening is a cost-effective means to rule out drug use in most samples, a positive screen must be confirmed for court admissibility. Samples that show a presumptive positive in Screening require a follow-up confirmation.

Should a sample display a presumptive positive during Screening, additional hair is removed from the original specimen for Extraction. During this phase, drugs are extracted at significantly lower concentrations than other methods (such as urine or oral fluid), making hair drug screening a challenging methodology.

Confirmation of any positive screen result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positives are cleaned prior to confirmation, as needed. The entire lab process, from Accessioning to Confirmation, is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
