What it is, How it works

When collected near the scalp, hair can offer a detection period of up to approximately 3 months for alcohol and various drugs. Hair is easy to gather, relatively hard to tamper with, and simple to transport.

A sample measuring 1.5 inches consisting of approximately 200 hair strands (comparable in size to a #2 pencil) closest to the scalp will yield 100mg of hair, the optimal sample for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advised. It's recommended to weigh the sample on a jeweler’s scale. If scalp hair is not available, a similar quantity of body hair can be collected. When referencing head hair, we mean scalp hair exclusively. Body hair refers to other types of hair (facial, axillary, etc.).

Process Overview

The four primary steps involved in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample within a laboratory’s system. This process includes verifying that the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening involves an initial rapid check for substance abuse. While Screening is a cost-effective method to exclude drug use in the majority of samples, a positive screen must be confirmed for it to be admissible in court. Any samples that test presumptively positive in Screening necessitate secondary confirmation.

In the event a sample tests presumptively positive during Screening, additional hair is taken from the initial specimen and prepared for Extraction. Here, drugs are extracted from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive screening result occurs via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleaned prior to confirmation as required. The entire laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.
