For EtG, additional assessments, or tests exceeding 10 panels, it is advisable to collect 150mg of the sample. We suggest using a jeweler's scale for specimen measurement. Should scalp hair be inaccessible, a corresponding quantity of body hair may be obtained. The phrase "head hair" specifically refers to scalp hair, whereas "body hair" encompasses all other hair types (such as facial or axillary hair).

Process Overview

The laboratory processing of drug test results includes four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial registration of a sample within a lab system. It involves checking that the sample was properly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and inputting any extra data not captured by an electronic chain of custody system.

Screening carries out a preliminary rapid assessment for drug compounds. Although Screening is a cost-efficient approach to exclude drug use for the bulk of samples, positive findings require further validation to be legally acceptable. Any samples flagged as presumptively positive during the Screening stage necessitate a subsequent confirmation.

Once a sample is tentatively positive at the Screening phase, additional hair is extracted from the original specimen for the Extraction phase. Here, drugs are isolated from hair at much lower concentrations compared to other techniques (e.g., urine or oral fluid), making hair drug testing a notably challenging procedure.

Confirmation of any positive Screening outcomes is performed through methods like GC/MS, GC/MS/MS, or LC/MS/MS. All tentative positive samples undergo washing before confirmation if needed. The comprehensive lab procedure, from Accessioning to Confirmation, is conducted under the CAP (College of American Pathologists) Hair designation and follows the accreditation standards of ISO/IEC 17025.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
