When cut near the scalp, hair can reveal up to a three-month period of alcohol and drug use. Collecting hair is straightforward, relatively tamper-proof, and conveniently shipped.

A 1.5-inch hair sample of approximately 200 strands (similar in size to a #2 pencil) closest to the scalp yields 100mg of hair, ideal for both screening and verification. For EtG, supplementary, and/or tests exceeding 10 panels, a 150mg sample is suggested. Utilizing a jeweler’s scale for measurement is recommended. Should scalp hair be unattainable, an equivalent amount of body hair can be used. The term head hair exclusively refers to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory procedure for drug testing involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails incorporating a sample into the laboratory's system, verifying its seal and proper shipment, assigning a unique LAN (Laboratory Accessioning Number), and completing any supplementary data input not captured by an electronic chain of custody system.

Screening provides an initial rapid analysis for drugs. Although it efficiently identifies potential drug use in most samples, a positive screening result requires confirmation for court admissibility. Presumptive positive samples from Screening necessitate a secondary verification.

For samples showing initial positive results during Screening, more hair is extracted from the original specimen and readied for Extraction. During this phase, the concentration of drugs extracted from hair is significantly lower than other methods (e.g., urine or oral fluid), making hair screening a challenging methodology to execute.

GC/MS, GC/MS/MS, or LC/MS/MS techniques confirm any positive screening results. Before Confirmation, all presumptive positive samples undergo washing as necessary. The entire laboratory workflow from Accessioning to Confirmation is overseen under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis.
