For tests evaluating EtG, additional panels, or exceeding 10 panels, 150mg of the specimen is suggested. Using a jeweler’s scale for weighing the specimen is recommended. If scalp hair isn't accessible, an equivalent quantity of body hair can be utilized. Head hair refers exclusively to scalp hair, whereas body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four crucial steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially integrating a sample into the laboratory’s system. This process includes verifying the proper sealing and shipment of the sample, attributing a random LAN (Laboratory Accessioning Number), and completing any necessary data entry absent in an electronic chain of custody system.

Screening initiates a preliminary assessment for drug abuse. While Screening is a budget-friendly manner to dismiss drug use in most samples, a positive result must be confirmed for legal acceptance. Samples with presumptive positive results during Screening require further confirmation.

If a sample is deemed presumptively positive in Screening, additional hair is extracted from the initial sample and prepped for Extraction. During Extraction, drugs are isolated from hair at concentrations significantly lower than those in alternative methods (ex. urine or oral fluid), rendering hair drug screening the most challenging technique to execute.

Positive results from Screening undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation when necessary. The comprehensive laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and accreditation per ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
