Process Overview

The laboratory analysis of a drug test result involves four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Any samples yielding presumptive positive results during Screening must undergo secondary confirmation.

Upon a presumptive positive outcome at the Screening phase, additional hair is extracted from the original specimen for Extraction. This procedure involves isolating drugs from hair at a notably lower concentration than other methods such as urine or oral fluid tests, making hair drug testing a challenging process.

Confirmation of any positive screening result uses techniques like GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens are subjected to cleansing before the confirmation stage as needed. The complete laboratory process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
